A 37 yr old male with seizures and evaluation assosiated with diabetes and hypertension

A 37 year old male who is a farmer and cable operator by occupation has presented to the casualty with chief complaints of Seizures on day before yesterday's night

HISTORY OF PRESENTING ILLNESS -

The patient was apparently asymptomatic 2 days back then he developed seizures while he was asleep and had 4 episodes (2 in home, 2 in ambulance)

he has a history of Headache since 8 days associated with generalised weakness. He also said he used to suffer with it earlier, but with less intensity.

There was Froathing but no tongue biting when he was brought to hospital.

On the day he developed seizures he ate an apple, pomegranate and a banana after which he consumed red wine.

There were Involuntary movements for 30 sec which first started in the left side and then became generalised.

There was Loss of consciousness for 3 min And no Post ictal confusion for 5 min 


PAST HISTORY-

There is History of diabetes mellitus (type 2) since 6 years and was using metformin 500mg OD ever since
No history of asthma, TB, HTN

FAMILY HISTORY-

Father is a known case of diabetes and hypertensive 
Elder brother- sudden death with chest pain on a night, before 2 years ago.

TREATMENT HISTORY-

Tab. Metformin 500mg + glimepiride 1mg OD in the morning 

GENERAL EXAMINATION-

Patient is conscious coherent and cooperative 
Well built and nourished
Temperature- 98.6*F      
Rr- 20cpm.                     
Spo2 -98%.  
Bp- 150/100mm hg               
Pulse-78bpm
Grbs-559  

Pallor - Absent
Icterus- absent 
Cyanosis - absent 
Clubbing- absent 
Lymphadenopathy- absent 
Edema- Absent

SYSTEMIC EXAMINATION-

CVS- s1 s2 heard, no murmurs 
RS - BAE+
CNS - intact
P/A - soft and non tender ,bowel sounds heard

INVESTIGATIONS-

Tests on 24 aug -

Tests done on 25 aug


PROVISIONAL DIAGNOSIS: seizures under evaluation with type 2 dm denovo hypertension 


TREATMENT -

1) inj HAI 1ml(400)+39 ml NS 
2)inj levipil 500mg iv bd 
3)inj thiamine 200mg in 100ml NS iv TID 
4)inj zofer 4mg/iv/sos
5)inj pan 40 mg iv OD 
6)BP/pr/rr/spo2 charting 2nd hourly 
7)tab ecosprin
8) tab atorvas 20 mg Po OD
